When to keep your child home
Required Health Forms
The following immunizations are required as a condition of attendance in all grades:
Diptheria –
Tetanus –
Poliomyelitis (Polio) – Three or more properly spaced doses.
Measles (Rubeola) –
Mumps –
German measles (Rubella) – One dose of vaccine administered at 12 months of age or older or history of immunity proven by lab testing.
Hepatitis B –
Chickenpox (varicella) – Two properly spaced doses of vaccine, the first dose at 12 months or older, or immunity proven by laboratory testing or written history of disease from parent, guardian or physician.
In addition to the above, student entering 7th grade must also have the following immunizations:
Tetnus and diphtheria and acellular pertussis vaccine (Tdap) – One dose if at least 5 years have elapsed since the last dose of a vaccine containing tetnus and diphtheria.
Meningococcal Conjugate Vaccine (MCV) – One dose of Meningococcal Conjugate Vaccine (MCV)
Physical form: State law requires this for school students in Kindergarten, 6th and 11th grades
Dental Form: State law requires dental exams to be performed in Kindergarten, 3rd, and 7th grades; a dentist from the school district will provide a dental evaluation when needed.
Scoliosis Form: Scoliosis evaluations are required in 6th and 7th grade as this is the time of greatest growth. In 6th grade it can be included on the physical form.
Required in 7th grade with students well visit from his/her primary care provider. Nurse will perform evaluation in school with parental permission on predetermined dates when needed.
If a student suffers any form of an accident (ie, concussion, cast, use of crutches, extremity in a boot), that results in being “excused from gym”, the nurse’s office needs a letter of documentation. This letter must come from the doctor’s office stating the reason and for how long they will be restricted. Also, we will need a letter when the student is able to return to gym class.
